Clinical Care Coordinator

We are looking for a highly experienced Clinical Care Coordinator. Candidates should have experience in acute inpatient case management and discharge planning. Training on the unit will be minimal, and candidates must be able to pick up quickly and work efficiently. The case load will consist of 15-20 patients per day. Consultations with the team will be available as needed. The facility reserves beds for complex placements, but specific criteria must be met for assistance. Candidates will receive a variety of complex patients due to the facility's status as a trauma hospital. Candidates must be able to dress in any color scrubs or business professional attire. This position requires rotational holiday coverage as needed. Candidates will work alongside a case management team and will not be alone during shifts.
