Your Specific Responsibilities:
The Health Educator Consultant plans and delivers health promotion and public health education programs for clinics, schools, and community groups. The position expands outreach, education, screenings, and referrals for chronic disease prevention, diabetes management, and tobacco cessation, and provides support to the Breast and Cervical Cancer Early Detection Program. This position supports the Diabetes Prevention Program (DPP) by assisting lead staff with program activities, supports the Tobacco/SWAT Coordinator by contributing to tobacco‑prevention policy efforts and youth empowerment education within the SWAT Chapter, and assists the Regional BCCEDP Coordinator as needed. The role also provides comprehensive support across all assigned programs, including outreach, documentation, reporting, meetings, and client coordination tasks, to ensure effective and seamless program operations.
Tobacco/SWAT Coordinator Support
- Plans, implements, and conducts health promotion and public health education programs for clinics, schools, and community groups.
- Serves as support for SWAT Coordinator. Serves as a member of the SWAT Chapter, assisting by demonstrating and teaching responsible youth empowerment through skill-building, increasing knowledge of tobacco issues and prevention, and providing advocacy training.
- May transport youth to statewide and regional events.
Breast and Cervical Cancer Early Detection Support
- Serves as backup to the Regional Coordinator for the Florida Breast and Cervical Cancer Early Detection Program, providing support in outreach planning, community engagement, participant recruitment, and promoting program awareness.
- Assists with client follow-up documentation in required tracking systems, ensures timely completion of monthly BCCEDP reports, and maintains program records in accordance with Department standards.
- Supports required program meetings, including monthly conference calls and biannual statewide meetings, and assists with client registration and service entry in HMS when needed.
Outreach Support
- Increases outreach and education among priority populations on chronic disease prevention and strategies to address associated risk factors.
- Increases screenings and referrals for diabetes prevention, diabetes management, asthma services, tobacco cessation programs, and supports the breast and cervical program.
Diabetes Prevention Program Support
Responsibilities may include obtaining certification as a Lifestyle Change Coach for the PreventT2 curriculum; participating in community outreach events to increase awareness and recruiting participants; assisting with DPP classes in both in-person and virtual formats; and developing and delivering presentations to small groups in various settings. Additional duties include participating in local, regional, and state meetings, training, and technical assistance calls; and assisting with the completion and submission of program reports on a monthly, quarterly, and biannual basis.
- Attend team meetings, CHD staff meetings and participate in staff development activities when appropriate.
- Maintains client and confidentiality in a manner consistent with Florida Statutes and DOH policy.
- Provides support for other initiatives and performs related duties as necessary with adjustable scheduling.
- Enter 100% of EARS within (1) day following the end of the pay period at least 97% of time. Records services performed into HMS immediately after providing services.
- Perform other duties as assigned by management.
Required Knowledge, Skills, and Abilities:
- Knowledge of the principles and practices of health education.
- Knowledge of variety of health specialty areas.
- Knowledge of the methods of data collection.
- Skill in operating a variety of audiovisual and computer equipment.
- Ability to plan and coordinate the development and implementation of health promotion and public health education programs and projects.
- Ability to collect and analyze data.
- Ability to evaluate health education practices for compliance with applicable statutes, rules and regulations.
- Ability to plan, organize and coordinate work assignments.
- Ability to communicate effectively.
- Ability to prepare a wide variety of written material such as reports, memoranda, pamphlets, curricula, lesson plans and grant requests.
- Ability to establish and maintain effective working relationships.
- Ability to understand and apply applicable rules, regulations, policies and procedures.
Qualifications:
This position requires daily in-office or field work.
Preferred – A bachelor’s degree from an accredited college or university and two years of professional experience in planning, evaluation, research, analysis, business, government, social health, or rehabilitation services or fiscal work.
A master’s degree from an accredited college or university can substitute for one year of the required experience.
A doctorate from an accredited college or university can substitute for the required experience.
Professional or nonprofessional experience as described above can substitute on a year-for-year basis for the required college education
