Job Title: Nurse Practitioner / Physician Assistant - In-Home Health Assessments


Location: Buffalo, NY ( Erie, Niagara, Chautauqua, Orleans Counties) - travel within ~50 minutes of home

Position Overview
We are seeking Nurse Practitioners and Physician Assistants to conduct Initial Health Assessments (IHAs) for Medicare and Medicaid members in Buffalo, NY . This role is primarily field-based , completing in-home patient visits with the flexibility to create your own schedule.


This opportunity is ideal for providers looking for supplemental income, flexible hours, or part-time clinical work . Assessments typically take 15-60 minutes and focus on preventive care, health history, and overall wellness evaluations.


Key Responsibilities

• Conduct comprehensive health histories and physical assessments in patients' homes
• Review medications and evaluate overall health status
• Identify social determinants of health and provide patient education
• Document assessments accurately in the EMR within 24 hours
• Travel to patient homes within a 50-minute radius

Compensation

• $550 for training
• $100 per in-home assessment
• $70 per telehealth assessment
• $50 compensation for no-shows

Schedule

• Fully flexible schedule - choose your own availability
• Minimum expectation of 30 assessments within the first month
• Opportunity for daytime, evening, or weekend visits

Qualifications

• Active Nurse Practitioner (NP/APRN) or Physician Assistant (PA) license
• At least 1 year of clinical experience
• Previous home health or Medicare assessment experience preferred
• Ability to travel locally for in-home visits
