Family Nurse Practitioner

A federally qualified health center in Kansas City is hiring an NP or PA who wants to practice medicine the way it was meant to be practiced — enough time with each patient, a real team behind you, and a direct line to clinical leadership when it matters. The PCMH model here means care coordination, chronic disease management, and pediatrics are built into how the practice runs, not squeezed between appointments. Spanish language proficiency preferred. Active, unrestricted Missouri APRN or PA license required in hand at application — no exceptions. Key Responsibilities Provide acute, chronic, and preventive primary care including history, physical exams, and prescribing Review and interpret labs, radiology, and consultations; follow up with patients directly Identify barriers to care and coordinate referrals to internal and community support services Provide ongoing patient education on diagnosis and care plan Document thoroughly including visit reason, history, medications, orders, referrals, and follow-up Participate in Medical Executive meetings, quality improvement, peer review, and committee work Collaborate with Chief Medical Officer and Chief Clinical Officer on service and program development Active, unrestricted Missouri FNP or PA-C license in hand at application (hard requirement; no exceptions) FNP-BC or PA-C certification required Graduate of accredited NP or PA program BLS certification required Commitment to PCMH model and collaborative, committee-based care Active Missouri DEA and BNDD licensure preferred Interest in chronic disease management and pediatrics preferred Schedule & Compensation $104,536–$130,670/year Full-time | Five 8-hour shifts | Monday–Friday Outpatient | FQHC | Kansas City, MO Immediate hire The application and hiring process for this position will be managed through NPHire, a nurse practitioner and physician assistant job board connecting APPs with top opportunities nationwide. #J-18808-Ljbffr NPHire
