Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings to create custom treatment plans.
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functional independence.
• Work in partnership with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes.
• Provide education to patients and their families regarding therapeutic objectives, discharge planning, and home exercises.
• Keep detailed documentation of evaluations, treatments, and progress according to facility and regulatory standards.
• Engage in care team discussions and interdisciplinary meetings to ensure seamless care coordination.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent accredited program.

Certifications

• State licensure as a Physical Therapist .
• CPR certification is preferred.
• Specialized certifications (such as in neurology or geriatrics) are advantageous, though not mandatory.
