RN Case Manager - Hospice

Coordinate and manage patient care, ensuring adherence to hospice policies and procedures. Provide direct nursing care, including symptom management and pain control. Educate patients and families on end-of-life care and available resources. Collaborate with interdisciplinary teams to optimize patient outcomes. Document patient progress and maintain accurate medical records. Participate in on-call rotations to provide after-hours support. Advocate for patients\' needs and preferences in care decisions. Requirements Current RN license in the state of Wisconsin. Minimum of 2 years of nursing experience, preferably in hospice or palliative care. Strong assessment, communication, and interpersonal skills. Ability to work independently and as part of a collaborative team. Compassionate demeanor with a focus on patient-centered care. Experience with electronic medical records (EMR) systems. Certification in hospice and palliative nursing (CHPN) is a plus. Willingness to travel within the service area as needed.
