Geriatric Social Worker - Patient-Centered Advocacy

Covenant Retirement Communities & Covenant Care

Covenant Living of Cromwell is seeking a Social Worker for a full-time role on-site in Cromwell, CT. You will assess the psycho-social needs of residents, provide counseling to residents and families, and coordinate discharge planning and admissions processes.

The role requires a BA or BSW in social work or related fields and state licensure, with emphasis on geriatric care. Ideal candidates will have at least two years in geriatric social work, strong English communication skills, and a warm,
