Clinical Care Coordinator - Neurosurgery

Job Description The mission of the Coordinator Clinical Care is to identify high risk patients and help coordinate resources to facilitate care, improve health outcomes, and reduce avoidable hospitalizations and re‑admissions. The goal is to provide the best quality medical care while working toward a patient‑centered medical home and to ease our patients' way. Required Qualifications Graduate of school for medical assistants and certificate. Coursework/Training: Diploma/certificate from the military in Medical Specialist Course. 2 years directed patient care at MA level or higher. Benefits Wages: $24.82 - $37.98 per hour. Additional compensation may be available for this role, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
