Continuum Therapy Partners is seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team at Sauk County Health Care Center in Reedsburg, Wisconsin.

This opportunity offers Full-Time, Part-Time, and PRN scheduling options for a licensed Physical Therapist who enjoys providing meaningful, patient-centered care in a supportive skilled nursing environment.

As a Physical Therapist (PT) , you will work with adult and geriatric patients to improve mobility, strength, balance, endurance, and functional independence while collaborating with an interdisciplinary rehabilitation team focused on quality outcomes.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance, and functional limitations.
• Develop and implement individualized physical therapy treatment plans.
• Provide skilled physical therapy interventions to improve movement, independence, and quality of life.
• Perform therapeutic exercises, gait training, mobility training, and functional activities.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and caregivers on mobility strategies, exercises, and safety techniques.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Wisconsin Physical Therapist (PT) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
• Passion for providing high-quality, patient-centered rehabilitation care.
