Continuum Therapy Partners is seeking a compassionate and dedicated Occupational Therapist (OT) to join our rehabilitation team at Sauk County Health Care Center in Reedsburg, Wisconsin.

This opportunity offers Full-Time, Part-Time, and PRN scheduling options for a licensed Occupational Therapist who enjoys providing meaningful, patient-centered care in a supportive skilled nursing environment.

As an Occupational Therapist (OT) , you will work with adult and geriatric patients to improve independence, safety, mobility, and quality of life through individualized occupational therapy interventions. You will collaborate with an interdisciplinary rehabilitation team committed to achieving positive patient outcomes.

Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy services in a skilled nursing facility setting.
• Address activities of daily living (ADLs), functional mobility, coordination, strength, cognition, and safety awareness.
• Help patients achieve greater independence and participate in meaningful daily activities.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and caregivers on therapeutic techniques, adaptive equipment, and safety strategies.
• Deliver compassionate, evidence-based occupational therapy services.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Wisconsin Occupational Therapist (OT) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
• Passion for providing high-quality, patient-centered care.
