Alpine is seeking a Population Health Specialist to support Colorado Community Health Alliance (CCHA) Medicaid members by driving performance on quality metrics. You will work with clinical operations, CCHA leadership, and health plan teams to close care gaps and coordinate outreach across settings. The role requires Colorado residency and 1–3 years of healthcare data/quality experience, with proficiency in Microsoft 365, EMR systems, and strong communication. #J-18808-Ljbffr Phpmcs
