RN Care Manager - Senior Care | Field-Based

C & S Care Management, part of the trusted C & S Healthcare Services family, is seeking a compassionate, dependable, and organized Registered Nurse (RN) Care Manager to support elderly clients and their families. This is a field-based, client-facing role involving regular local travel to clients’ homes, assisted living communities, rehabilitation facilities, hospitals, medical appointments, and other care-related settings. The RN Care Manager provides hands‑on care coordination, ongoing client support, and communication with families, caregivers, providers, and the internal care team. Position Overview The RN Care Manager serves as a trusted point of contact and dedicated advocate for elderly clients and their families. You will assess client needs, develop and update individualized care plans, coordinate services, and help ensure each client’s needs, preferences, safety concerns, and care goals are understood and supported. Responsibilities Conduct regular visits with elderly clients in homes, assisted living communities, rehabilitation facilities, hospitals, and other care settings. Assess client needs, preferences, routines, safety concerns, and changes in physical or cognitive condition. Develop, update, and monitor individualized care plans. Coordinate medical appointments, client transportation, and other support services; transport clients to medical appointments and care-related meetings as needed. Communicate care-plan updates and client needs with clients, families, caregivers, physicians, providers, and the internal care team. Monitor client well-being, satisfaction, and service quality; identify concerns and ensure appropriate follow-up. Maintain accurate documentation, including visit notes, care plans, client communications, and service updates Qualifications Current, active Registered Nurse (RN) license in the State of Texas required. Experience working directly with elderly clients, families, caregivers, home-care clients, or healthcare patients. Home health experience preferred. Experience in senior care, home care, care coordination, gerontology, or a related healthcare setting is a plus. Strong clinical observation, communication, organization, problem‑solving, and time‑management skills. Ability to work independently in the field while remaining connected to the office and care team. Ability and willingness to travel locally throughout the workday. Reliable transportation, valid driver’s license, and proof of auto insurance required. Comfortable using basic computer and mobile technology for documentation.
