SALARY RANGE: BU13 | SR20 SALARY RATE: $59,532.00 (Annual Salary) based on the FTE JOB DUTIES: This position is regional and is located in the Social Work Services department of the Hawai'i Health Systems Corporation (HHSC) Kaua'i Region, a public benefit corporation of the State of Hawai'i. The HHSC Kaua'i Region encompasses Kaua'i Veterans Memorial Hospital (KVMH) and Samuel Mahelona Memorial Hospital (SMMH), both designated as critical access hospitals (CAH), as well as the Kaua'i Region Clinics and its affiliated entities. The Social Work Services department provides social services to patients and families, conducts home visits, participates in referral, transfer, and discharge conferences, prepares necessary transfer and discharge documents, performs financial assessments (means tests) to determine the ability to pay, processes required documents for eligibility for admission into the Intermediate Care Facility, and maintains liaison with the Department of Human Services and other governmental agencies. The Social Worker III is responsible for delivering crisis intervention, psychotherapy, family therapy, and comprehensive case management services for assigned patients/residents and their families. This position may also lead or assist with the referral and admission process for new cases and provides technical guidance to physicians, hospital staff, and outside agencies on methods and procedures for handling specific cases in accordance with State and Federal guidelines. The position requires travel to and from other facilities within the region and may be subject to working extended hours, weekends, holidays, etc., to meet operational requirements and patient care needs. This position reports to the Registered Professional Nurse VI (Regional Director of Quality & Compliance Integrity), position number 60734, or their designee. THE MINIMUM QUALIFICATION REQUIREMENTS ARE: Applicants must meet all of the following requirements. Please note that unless specifically indicated, the required education and experiences may not be gained concurrently. In addition, qualifying work experiences are based on a 40-hour work week. EDUCATION: Graduation with a bachelor's degree or master's degree in social work from a program of study accredited by the Council on Social Work Education, or a doctoral degree in social work from a college or university accredited by the Western Association of Schools and Colleges, or a comparable regional accreditation body. A master's degree from a social work program accredited by the Council on Social Work Education is preferred. SPECIALIZED EXPERIENCE: One (1) year of progressively responsible professional work experience, which involved helping individuals and their families find satisfactory ways of identifying their problems, coping with their conditions, and functioning effectively within their environments. Depending on the employment setting and the kinds of clients or patients served, job duties and responsibilities may vary, although typically the work will include the identification and evaluation of the client's or patient's problems and needs; the development of a service or treatment plan, sometimes in tandem with other professionals working in an interdisciplinary team; the initiation and implementation of the service plan, either directly or through the authorization of provider/vendor services; monitoring of services being provided; and evaluation/assessment of the client's or patient's progress, with amendments to the service/treatment plan made as appropriate. One (1) year of professional experience in a healthcare setting is preferred. LICENSURE: Possession of a State of Hawai'i type #3 driver's license preferred. DESIRED SKILLS:
