Physical Therapist - Post-Acute Setting

We are seeking a skilled and compassionate Physical Therapist to join a post-acute care facility in Modesto California. This role focuses on working with patients recovering from surgery illness injury and medically complex conditions. You will play a key role in helping patients regain mobility improve strength and endurance and achieve greater functional independence. Job Responsibilities:

Evaluate diagnose and treat patients with physical impairments in a post-acute setting

Develop and implement individualized treatment plans based on patient needs and clinical findings

Collaborate with interdisciplinary teams to ensure coordinated patient care and recovery planning

Supervise and work closely with Physical Therapist Assistants as needed

Monitor patient progress and modify treatment plans based on clinical outcomes

Educate patients and caregivers on mobility safety and home exercise programs

Maintain accurate and timely documentation in compliance with facility and regulatory standards

Requirements

Active state license as a Physical Therapist in California

Current BLS/CPR certification

Minimum of 1 year of recent PT experience preferred ideally in post-acute SNF or rehabilitation settings

Strong clinical reasoning and patient care skills

Ability to work collaboratively within a multidisciplinary healthcare team

Required Skills:

Job Requirements: Active state license as a Speech-Language Pathologist in California ASHA CCC-SLP certification preferred or required depending on facility policies Current BLS/CPR certification preferred 01 year of experience welcome (CFY candidates and new graduates encouraged to apply) Experience or clinical exposure in acute care dysphagia or medically complex patients is a plus Strong communication clinical reasoning and patient care skills
