Clinical Supervisor: Nursing

Full‑Time Clinical Supervisor: Nursing Anticipated Coverage area: Sonoma, Marin, Napa, Solano In this leadership role you will coordinate patient care and management by a team of Registered Nurse (RN) Case Managers and LVNs. You will serve as the first point of escalation for support and will design nursing educational programs, evaluate their effectiveness, and maintain relationships with customers through updates such as readmission reports. Working schedule: 2 days per week in the office for administrative and team‑management duties; 3 days per week (≈12 visits per week) in the field supervising RN Case Managers. Responsibilities In‑field oversight of clinicians who provide care and treatment to patients in their homes. Handle incoming calls promptly and follow up to ensure appropriate actions are taken for internal and external customers. Coordinate training and educational programs for field nurses to improve clinical outcomes and competencies. Assess staff learning needs, formulate specific plans and goals for the next evaluation period, and serve as a role model and resource for staff. Act as the first point of contact for field clinicians to assist with patient‑care escalations. Coordinate at least bi‑weekly case conferences with the clinical team to discuss patient progress and revise care plans as needed. Review and evaluate each case by reviewing services provided by clinicians, conferences, record review, and instructing clinicians to promote effective performance and quality home‑care delivery. Review patient’s clinical diagnosis, medications, procedures and clinical course. Assist clinicians in establishing immediate and long‑term therapeutic goals, setting priorities, and developing a plan of care. Qualifications Two years of management or supervisory experience within the past five years in a health‑care setting, preferably in home health. Graduate of an accredited school of nursing; Bachelor’s degree preferred. Current California Registered Nurse (RN) license. Current Basic Life Support (BLS) certification. Valid California driver’s license with an acceptable driving record. All licenses and certifications must be current at the time of hire and maintained throughout employment. Benefits Competitive compensation ranging from $155,000 to $170,000 per year. Generous annual bonus up to $40,000, paid quarterly. 401(k) with up to 4% match.
