Acute Care Case Manager Discharge Planning

Schedule

Weekend requirement: Every other weekend; must be available for holidays. Holiday commitment: Must commit to working 3 of the following 5 Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day.

Assignment Details

Assignment type: Acute care hospital. Daily caseload: up to 25 patients. Orientation: 40 hours.

Requirements

California state license required; must be active by start.
2 years of acute care case management or discharge planning experience required.
Travel experience required; first-time travelers may be considered after screening.
InterQual experience highly preferred.
Key clinical skills include care coordination, discharge planning, utilization review, and knowledge of Medicare/MediCal and payer systems.
Experience with advance directives and behavioral/mental health crisis navigation preferred.
Charting system: Epic.

Responsibilities

Coordinate patient care and discharge planning activities.
Conduct utilization reviews and determine medical necessity.
Arrange durable medical equipment, skilled nursing facility placements, and ambulance transport as needed.
Navigating aspects of protective services and behavioral health crises.
Provide documentation in computerized systems.
Collaborate with interdisciplinary teams to ensure comprehensive patient support.

Compliance / Notes

Pre-approved time off limited to 1 request per contract; maximum of 7 days. Background checks required.
