RN Hospice Care Liaison & Referral Lead

Bgcarenav seeks an RN liaison to educate hospital staff on hospice services and programs, coordinating referrals and ensuring smooth transitions of care. You will oversee case management for hospitalized patients and collaborate with physicians and interdisciplinary teams, reporting to the Executive Director.
