As we continue to grow, we are seeking a dedicated, experienced Certified Nurse Practitioner to join our Family Health Center team and help expand access to comprehensive, patient-centered care. Position: Certified Nurse Practitioner (Full-time or Part-time) Location: [Insert Clinic Location - e.g., Abington, Norristown, Pottstown or other VNACS Family Health Center site] Reports to: Clinic Medical Director / Family Health Center Leadership What You'll Do Provide comprehensive primary care services to patients of all ages (infants through seniors), including wellness exams, acute care, and chronic disease management. Conduct patient assessments, diagnose illnesses, order/interpret diagnostic tests, and develop and implement treatment plans. Prescribe medications and therapies, including managing ongoing medication regimens. Provide patient education, counseling, preventive health guidance, and support for health maintenance. Collaborate closely with other healthcare professionals - including RNs, support staff, and administrative team - to ensure coordinated, high-quality care. Maintain accurate patient records and documentation, including use of electronic health record (EHR) systems. Participate in ongoing professional development, training, and quality-improvement initiatives to support our mission of compassionate, patient-centered care. Who You Are - Required Qualifications Master's or Doctoral degree in Nursing (or equivalent) from an accredited Nurse Practitioner program. Active Nurse Practitioner license in the state of Pennsylvania (or relevant state licensure) and national certification (e.g., family practice or adult-gerontology). Strong clinical judgment, diagnostic ability, and comfort managing a broad spectrum of health conditions and patient populations. Excellent communication skills, with a patient-centered and compassionate approach. Capacity to work independently and collaboratively as part of a multidisciplinary care team. Familiarity with electronic health records (EHR) and documentation best practices.
