Overview Home-HealthMed Services, LLC is a Virginia-licensed, DMAS-enrolled Home Care Organization (HCO) providing skilled and non-skilled home care services across the Hampton Roads region. We are a certified small woman-owned minority-owned (SWaM) business committed to delivering person-centered care in compliance with 12VAC5-381 and 12VAC30-50-160. We are building a network of highly skilled, independent RN contractors to support our growing caseload throughout all eight service localities. Responsibilities Conduct in-home nursing assessments for new and existing clients per 12VAC30-60-70 and 12VAC30-120-930. Complete all required clinical forms, care plans, and DMAS-mandated documentation. Submit finalized documentation within 24 hours of each completed visit. Communicate accurate clinical findings to the Director of Nursing (DON) as required. Maintain client confidentiality in full compliance with HIPAA and Virginia privacy regulations. Adhere to all Home-HealthMed Services policies and applicable Virginia HCO licensure standards. Qualifications Active, unrestricted Virginia RN license (in good standing with the Virginia Board of Nursing). Minimum 1 year of RN clinical experience. Strong clinical assessment and documentation skills. Reliable transportation and ability to travel within your designated service area(s). Proficiency with electronic documentation (tablet or laptop required). Preferred: 2+ years of RN experience. Preferred: Home health, community health, or case management background — strongly preferred. Preferred: Familiarity with Virginia DMAS regulations and Medicaid-funded home care services. What We Offer $100 – $130 per completed assessment — competitive Hampton Roads market rate. Weekly Net-7 payment — get paid fast, not Net-30 like most agencies.
