Data-Savvy Community Health Program Coordinator

AltaMed

AltaMed Health Services Corporation is seeking a Community Health Program Coordinator to identify high-risk patients and coordinate referrals, eligibility checks, and CHW caseload monitoring. The role supports data collection and program evaluation for an evidence-based health care program serving low-income, high-risk patients. The ideal candidate will have 3+ years of data-related experience and familiarity with EMR systems, and is committed to advancing community health outcomes through #J-18808-Ljbffr AltaMed Health Services Corporation
