Housing Case Manager

The Housing Case Manager position pays $47,000 to $51,000/year, dependent on skills and experience, offers a generous benefits package that includes excellent health, dental, vision, and hearing insurance as well as paid time off that includes 15 days of vacation per year, 12 sick days per year (6 of which can be taken like vacation days), 12 paid holidays, 2 personal days, and the ability to earn an additional 2 days of PTO through our wellness program. Interview and ascertain the needs of applicants seeking housing assistance and determine program eligibility. Provide ongoing case management to clients by developing and monitoring individualized case plans designed to help clients achieve the highest level of self-sufficiency as they move through the Continuum of Care. Provide follow-up case management services for a period of at least one year. Identify and utilize additional community resources that will help clients. Refer clients to these services. Record all data in electronic client management database. Maintain financial guidelines when providing assistance in order to stay within budgeted allotments. Reports monthly statistics and annual outcomes to the Director of Social Services. Build and maintain rapport with community agency representatives and housing providers to ensure collaboration within the community. Attend monthly Brown County Homeless and Housing Coalition meeting, Brown County Social Services Providers meeting, Coalition subcommittee meeting, and other meetings as assigned. Participates in professional development opportunities related to position. Participates in seasonal activities/special events as requested by supervisor or Salvation Army Officer(s). Minimum Qualifications: The requirements listed below are representative of the minimal education, experience, skills, and/or abilities required for this position. Education: Bachelor's Degree in Social Work or related field. Experience: Two years case management experience with experience in mental health and/or AODA experience preferred -OR- Any equivalent combination of training and experience which provides the required knowledge, skills and abilities. Certifications: Valid Wisconsin Driver's License with clearance to drive from TSA's insurance carrier. Must obtain CPR/AED/First Aid with Naloxone certification for Adults, Children, and Infants within 60 days of hire. Skills/Abilities: Knowledge of: Principles and practices of social work and case management techniques relevant to the services offered. Federal regulations (including HUD) and state laws applicable to the programs. Appropriate interviewing skills. Public and private social services providers within the community and community resources and programs available to clients with identified needs. Collaborative case planning. De-escalation and crisis intervention techniques. Computer applications related to the work including skills in Microsoft Word and Excel, Internet and email. Record keeping principles and practices. Techniques for dealing with a variety of individuals from various socio-economic, ethnic and cultural backgrounds, in person and over the phone. Programs and eligibility requirements for the Social Security Administration, State Medicaid and Badger Care, County Social Services, Foodshare and other relevant programs. Ability to: Adhere to appropriate boundaries and ethics. Demonstrate sensitivity with clients of diverse socioeconomic backgrounds. Maintain confidentiality of information. Analyze, interpret, apply and explain applicable complex laws, codes, regulations and procedures. Assess cases appropriately and utilize most appropriate community resources to provide effective client services. Use initiative and independent judgment within established procedural guidelines; organize own work, set priorities and meet critical deadlines. Establish and maintain effective working relationships with those contacted in the course of work. Communicate exceptionally well orally, in writing and over the telephone.
