(14848) Continuity of Care Liaison

Direct Care - Clinical Therapist/Social Work/Case Worker Bridging Gaps in Healthcare—Join Us in Transforming Lives The Continuity of Care Liaison ensures uninterrupted, person-centered transitions between inpatient and community services by coordinating admissions, discharges, and post-discharge supports across State Mental Health Facilities (SMHFs), Contracted Psychiatric Beds (CPBs), Residential Treatment Centers (RTCs), and community providers. This role leads communication, scheduling, and linkage to services, to ensure individuals have timely follow-up care, medications, and support that promote recovery and community stability.
