Palliative Care - Nurse Practitioner

Palliative Care Pediatric Nurse Practitioner We are seeking a Nurse Practitioner for our Palliative Care Division. The candidate will function in an advanced practitioner role as a clinical expert in the management of pediatric patients appropriate for palliative care within a family-centered care environment. The nurse practitioner will assess, evaluate and make recommendations for the management of children with serious and complicated illnesses, under the direction of the palliative care physician and in collaboration with the palliative care team. The position involves activities in both inpatient and outpatient settings with a heavy focus on symptom management and care coordination. Responsibilities Assess, evaluate and manage pediatric patients eligible for palliative care in collaboration with the palliative care team. Provide advanced practice care under standardized procedures and process protocols. Support patients and families with medical decision making and throughout the continuum of care, including end-of-life considerations. Collaborate with other professional disciplines to ensure safe, effective, and cohesive delivery of care aligned with patient and family wishes. Provide staff support and education; coordinate patients’ care; and contribute to the development of policies and procedures for palliative care in both in- and outpatient settings. Serve as a consultant, educator and role model for CHOC and community health professionals. Qualifications California licensed Nurse Practitioner with at least one year of experience in direct pediatric patient care. Excellent communication skills and emphasis on effective interpersonal interactions. While preferred, prior pediatric palliative care experience and robust symptom management experience (pharmacologic and non-pharmacologic) are not required; in-depth knowledge of communication and emotional intelligence is necessary. Ability to work in a setting that includes both inpatient and outpatient care and to support patients and families through medical decision making and care coordination.
