Work Environment

This position is fully remote, allowing you the flexibility to perform your role from anywhere while engaging effectively with clients, families, and interdisciplinary teams. As a member of our hospice care team, you will leverage digital communication tools and health management software to ensure seamless interactions with clients and healthcare professionals. Key Responsibilities

• Liaison Work: Serve as a pivotal link among clients, families, and outpatient providers, ensuring cohesive care planning and communication throughout the therapeutic process.
• Insurance Coordination: Facilitate interactions with insurance companies for pre-certification and utilization management, advocating on behalf of clients to streamline their access to necessary services.
• Discharge Planning: Collaborate in discharge and aftercare planning, helping clients and families navigate post-treatment resources and support systems.
• Therapeutic Meal Support: Provide guidance and support during therapeutic meals, modeling healthy relationships with food and discussing the emotional aspects of eating and nutrition.
• Client-Centered Care: Deliver safe, empathetic, and affirming support tailored to each client’s unique circumstances, taking into account their intersecting identities and personal histories.

Patient Population / Client Focus

Our primary focus is on individuals undergoing hospice care, including older adults facing life-limiting illnesses. You will engage with clients and families dealing with grief, anxiety, and existential concerns associated with end-of-life transitions. Your expertise will be instrumental in addressing both the emotional and social needs of these clients, fostering dignity and enhancing their quality of life during challenging times.

Qualifications

• Education: Candidates must possess a Master's degree in Social Work from an accredited institution.
• Licensure: A valid Licensed Clinical Social Worker (LCSW) license in the state of New York is essential for this role.
• Clinical Skills: Strong clinical and diagnostic capabilities are required to assess client needs effectively.
• Communication: Excellent communication and interpersonal skills are crucial for fostering effective relationships with clients, families, and colleagues.

Compensation and Benefits
