Work Environment\n

This role will be based remotely within the context of our Hospice Center services, offering flexibility while maintaining a strong commitment to patient care. You will collaborate with a diverse team of professionals through virtual meetings, utilizing various digital communication tools to facilitate discussions and case reviews. This remote setting allows for a balance of autonomy in your work while still contributing to meaningful outcomes for children and families.

Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children's developmental status, utilizing standardized tools and observational methods.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP), ensuring that the strategies align with each child's unique needs.
\n
• Monitor child and family progress through regular follow-ups, adapting interventions and supports to optimize outcomes.
\n
• Foster positive relationships with children and their families, helping create a nurturing environment conducive to learning and growth.
\n
• Compile and maintain accurate documentation to ensure compliance with Federal and State regulations while upholding the highest ethical standards.
\n
• Administer evidence-based strategies during interventions, applying the Natural Learning Environment Practices to maximize children's development.
\n
• Provide guidance and support to families, equipping them with resources, skills, and strategies to enrich their child's developmental journey.
\n
• Stay current with advancements in early intervention methodologies and collaborate closely with other team members, exchanging insights and best practices.

Patient Population / Client Focus\n

The focus of this position includes children from the youngest ages up to three years old, who may have various disabilities or developmental delays impacting their physical, cognitive, social/emotional, and communication skills. The LCSW will utilize a strengths-based approach to engage with these children and their families, providing tailored support that acknowledges each family's unique circumstances and needs.

Qualifications\n

• A Master's Degree or Doctorate in Social Work is required.

• Active licensure as an LCSW from the Department of Public Health is mandatory.
\n
• A valid driver’s license is necessary for travel to client homes, as appropriate.

• A minimum of one year's experience in early intervention services, particularly working with children and their families.
