Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to understand the developmental needs of children ages birth to three.
\n
• Actively participate in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring alignment with each child's developmental milestones and family priorities.
\n
• Collaborate effectively with families to create joint plans that utilize evidence-based strategies within natural learning environments.
\n
• Monitor and evaluate outcomes as part of a multidisciplinary team, adjusting interventions as necessary to optimize child development.
\n
• Utilize a coaching model to educate families, providing them with the skills and tools needed to support their child's development in real-life contexts.
\n
• Ensure compliance with all Federal and State regulations related to social work and early intervention services.
\n
• Maintain detailed and accurate documentation of assessments, plans, and case notes in accordance with best practices and regulatory standards.

Patient Population / Client Focus\n

Your primary focus will be on infants and toddlers who may exhibit developmental delays or disabilities. You will provide critical early intervention services that empower families and support their children’s growth in various developmental areas, including cognitive, physical, social-emotional, and communication skills. Each interaction contributes to nurturing an inclusive and supportive environment, helping families engage meaningfully with their child's growth.

Qualifications\n

• Master's Degree or Doctorate in Social Work.

• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) through the Department of Public Health.

• A valid driver's license is required for travel to community settings as needed.
