Key Responsibilities\n

• Conduct thorough psychosocial assessments via telehealth to identify clients' strengths, needs, and therapeutic goals.

• Provide evidence-based therapy techniques, such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), to individuals and groups, understanding unique case dynamics.

• Engage in multidisciplinary collaboration with healthcare providers to create integrated care plans that address both physical and mental health matters.

• Advocate for clients by facilitating access to community resources and services that support their recovery and well-being.

• Maintain accurate documentation of client interactions, treatment plans, and progress outcomes using our electronic health records system.

• Participate in regular team meetings and case conferences to refine clinical skills and share best practices with peers.

Patient Population / Client Focus\n

You will work closely with diverse populations, including individuals coping with chronic illnesses, mental health disorders, and those requiring end-of-life care due to hospice services. Your role will entail addressing complex emotional and social issues, providing a holistic approach to care, and promoting resilience in challenging circumstances.

Qualifications\n

• A Master's degree in Social Work from an accredited program is required for this role.

• Must hold a current licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington.

• A minimum of two (2) years of post-licensure clinical social work experience is necessary.

• Strong competency in conducting diagnostic assessments and developing personalized treatment plans based on individual needs.
