Work Environment\n

This position operates within a community health clinic setting, though work is conducted remotely. You will have the flexibility to manage your schedule while working with a supportive team of professionals. Key Responsibilities\n

• Conduct thorough evaluations and assessments of children to identify developmental needs and strengths.

• Develop and implement Individualized Family Service Plans (IFSP) in collaboration with families and multidisciplinary team members.

• Monitor and document progress, adjusting intervention strategies as necessary to support children’s developmental milestones.

• Provide targeted support to families, equipping them with information, skills, and strategies to enhance their child's development within natural learning environments.

• Engage in ongoing communication with families, addressing their concerns and ensuring they are integral to the decision-making process regarding their child's care.

• Adhere to all Federal and State regulations regarding service provision while maintaining a commitment to upholding the highest standards of health and safety.

• Utilize evidence-based practices tailored to the unique needs of each child, fostering a nurturing learning environment.

• Facilitate a coaching model to impart effective teaching strategies to families, enhancing their capabilities to support their child’s growth.

Patient Population / Client Focus\n

This role primarily focuses on children from birth to three years of age, particularly those with disabilities or developmental delays. You will interact closely with families, providing them direct support and guidance, ensuring that they understand the developmental processes and how to actively engage in their child’s growth. Your work will contribute significantly to the overall health and well-being of these vulnerable populations.

Qualifications\n

• A Master’s Degree or Doctorate in Social Work from an accredited institution.

• Current licensure as a Clinical Social Worker (LCSW) by the Department of Public Health is required.
