Job Details:

• Location: Lebanon, Pennsylvania
• Schedule: 8-hour days, Monday through Friday
• Minimum Hours Per Week: 40 hours

• Required Qualifications:
  • Certification in Basic Life Support (BLS)
  • Valid license to practice as a Physical Therapist in Pennsylvania
  • 1 year of clinical experience in home health or outpatient therapy
  • EMR proficiency with Epic system preferred but not mandatory
• Preferred Qualifications:
  • Experience with OASIS documentation
  • Ability to care for COVID-19 patients
• Additional Information:
  • The facility is tobacco and fragrance free; prior notification required.
  • May support neighboring counties, primarily western York and Adams.
  • Supporting documentation for work hours and mileage must be uploaded with submissions.
  • Vaccinations: COVID-19 and Flu vaccines are required; medical or religious exemptions are permissible.
  • The position involves patient care for adults and geriatrics, focusing on mobility, gait training, and patient education.
