PRN Care Manager

Position Overview The Care Manager plays a vital role within an interdisciplinary team, assessing, planning, advocating, and coordinating patient care from admission to discharge to ensure a safe and seamless transition after hospitalization. They focus on maintaining patient safety, quality of care, and positive patient experience by overseeing length of stay and implementing best practices using performance metrics. Responsibilities Assess patient needs and develop individualized care plans Coordinate care transitions to optimize safety and quality Advocate for patients throughout their hospital stay and post-discharge Utilize performance metrics and best practices to improve outcomes Qualifications & Skills Graduate of an accredited School of Nursing; Bachelor's Degree in Nursing required 2-3 years of experience in an acute care hospital setting preferred Current state licensure required Basic Life Support (BLS/CPR) certification required or obtained within 90 days Care Management or Utilization Management experience and certification preferred #J-18808-Ljbffr Mental Health Association of Rochester
