Additional Details about the Position
- PPD 4-8
- Providing specialized case management and care coordination for patients with special health care needs.
- Reviewing and monitoring outpatient consultations and referrals for medical necessity, cost effectiveness, and appropriateness.
- Collaborating with PRC staff, providers, and external facilities to ensure care aligns with CMS criteria.
- Maintaining statistical data on consults, monitoring deferred referrals, and supporting PRC team continuous quality improvement.
Experience Requirements
- IHS Purchased Referred Care (PRC) case management experience is a must
Certification Requirements
- BLS
- ACLS
- PALS
- RN
Licensure Requirement
- Any State License

Our Health Center is a modern ambulatory care center that was built in 2007 and is located 70 miles west of Oklahoma City in Clinton, Oklahoma. The facility is accredited by The Joint Commission and is a certified Primary Care Medical Home.
