Minimum Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program recognized by the APTA.
• An active Physical Therapy license in Illinois is essential to begin this role.
• Minimum educational requirement: Bachelor's degree.
• BLS certification from AHA or ARC may be necessary.
• Experience or capability to conduct in-home physical therapy evaluations and interventions.
• Must be able to provide coverage across Williamson, Jackson, and Franklin counties.

Preferred Qualifications

• Experience in home health settings is preferred.
• Familiarity with OASIS documentation is advantageous.
• Experience with Homecare Homebase (HCHB) is a plus.

Assignment Details

• Shift hours: TBD.
• Weekly hour guarantee: 32 hours.
• No on-call or weekend requirements.
• Mileage will be reimbursed for travel between patient visits, from the first to the last appointment.
• Service areas include Williamson, Jackson, and Franklin counties.
