Optum Home & Community Care is seeking a Telehealth Advanced Practice Clinician (APC) to provide virtual care for patients across settings and to coordinate care with caregivers, facilities, and providers. This per diem, work-from-home role offers flexible hours and holiday coverage while maintaining patient privacy and secure EMR use. The APC must hold NP/FNP/Gerontology credentials with active state licensure, DEA authority or ability to obtain, and board certification. #J-18808-Ljbffr Chicago Derm
