Job Description

Job Summary:
The Case Manager is responsible for coordinating patient care, ensuring compliance with healthcare regulations, and facilitating smooth communication among medical teams. They play a vital role in optimizing patient outcomes through clinical oversight and resource management.

Responsibilities:

• Assess patient needs and develop individualized care plans.
• Collaborate with healthcare providers to coordinate services and monitor progress.
• Ensure documentation and compliance with healthcare standards and policies.
• Facilitate patient education and discharge planning.
• Maintain accurate and timely records of patient care activities.
• Navigate regulatory requirements related to travel and licensing for agency assignments.

Qualifications:

• Must possess an active nursing license and pending or current NM/Compact license.
• Previous experience in case management or hospital nursing preferred.
• Ability to work independently and respond swiftly to patient needs.
• Strong organizational skills with attention to detail.
• Knowledge of healthcare compliance, travel contracts, and documentation protocols.
• Ability to float to related units within scope as needed.
