Requirements
- Minimum experience: 1 year
- Certifications: BLS
- Skills: Care coordination, discharge planning, admission criteria, use of InterQual and Milliman guidelines, knowledge of CMS, CPT, DRG, HEDIS, NCQA standards, HIPAA, OSHA, The Joint Commission/core measures, workers' compensation, ICD-10 coding familiarity.

Roles & Responsibilities
- Coordinate patient care and discharge planning in collaboration with the healthcare team.
- Assess and manage admission criteria and patient needs throughout their care journey.
- Ensure compliance with regulatory standards, including CMS and HEDIS.
- Utilize electronic charting systems to document care effectively and accurately.
- Provide education and resources to patients and families to support post-discharge care.

Compliance / Notes
- Background checks required for travel assignments.
- Orientation modules to be completed onsite.
- All requests for time off must be listed at the time of submittal; up to 4 days can be auto-approved.
