Case Manager

This is a full-time, hybrid (4 days onsite, 1 day remote) M-F, 9am-5pm role located in Providence, RI, for a Case Manager. The Case Manager will be responsible for coordinating and managing care plans for elders and long term support services, acting as a liaison between clients and service providers, and ensuring clients receive appropriate resources and support.
