Role & Responsibilities Serve as a leader within the healthcare team by establishing personalized patient relationships. Collaborate with physicians and team members to assess, plan, implement, and evaluate individualized care plans. Utilize best scientific evidence and compassion to support patients' health journeys. Maintain responsibility, authority, and accountability for care management in accordance with policies and procedures. Qualifications & Skills Graduate of an accredited nursing school; Bachelor's degree preferred. At least one year of acute nursing experience. Current RN licensure in the appropriate state.
