RN Care Manager - Hybrid | Greater Boston Area

What You'll Do As an RN Care Manager, you'll serve as a trusted partner to caregivers by providing guidance, education, and support. Conduct in-home visits within your assigned territory to assess home safety, evaluate changes in health status Develop meaningful relationships with caregivers to promote confidence, reduce burnout, and improve outcomes. Implement individualized care plans and provide ongoing support through our caregiver platform, including education, resource referrals, and reinforcement of best practices. Collaborate with caregivers and families to identify needs and connect them with appropriate community resources. Maintain accurate and timely documentation while ensuring compliance with regulatory and accreditation standards. Participate in special projects and other duties as assigned. Travel Requirements This role requires travel throughout the Greater Boston market, including: US-MA-Boston | US-MA-Cambridge | US-MA-Waltham | US-MA-Surrounding Areas What You'll Bring Active Registered Nurse (RN) license in the state of Massachusetts. Minimum of two years of recent experience providing direct care to older adults or individuals with disabilities. Experience with care management, care planning, assessments, and supporting individuals with complex medical needs. Valid driver's license, reliable transportation, and automobile insurance. Strong communication and relationship-building skills. Proficiency with Microsoft Office and web-based applications, including care management systems.
