Registered Nurse Care Manager

Job Summary AtlantiCare is seeking a Nurse Case Manager to utilize advanced clinical skills and disease‑specific knowledge to guide identified patients through the continuum of care. The Nurse Case Manager conducts daily assessments to identify high‑risk patients and manages their care during acute care episodes, ensuring quality, cost‑effective services that support organizational goals. Responsibilities The Nurse Case Manager will: Conduct daily patient assessments to identify high‑risk individuals. Coordinate and manage patient care during the acute care episode. Provide quality customer service and participate in performance improvement initiatives. Demonstrate teamwork, cooperation, and commitment to organizational values. Qualifications Graduate of an accredited school of nursing (required). Bachelor’s in Nursing preferred. Current licensure as a Registered Nurse in the State of New Jersey. Basic Life Support Hand‑held (BLSHCP) Certification required within six months of hire. Masters degree in Nursing and advanced practice nurse certification may be required for certain specialties. 5–7 years of clinical experience (required). Prior case management or insurance management experience preferred. Excellent interpersonal, written communication, analytical, delegation, and computer literacy skills. Proficiency in clinical applications preferred at hire; required within 30‑60 days. Performance Expectations Demonstrate the technical competencies as established on the Assessment and Evaluation Tool. Work Environment The role involves frequent walking and standing with potential exposure to blood‑borne pathogens, infectious diseases, hazardous substances, and other hospital‑environment risks. Reporting Relationship The Nurse Case Manager at the Mainland site reports to the Case Management Divisional Manager, while the City site reports to the Case Management Director.
