Case Management Nurse

As a Case Manager RN, you will play a critical role in coordinating patient care across the continuum, ensuring safe transitions, effective resource utilization, and positive patient outcomes. You will work closely with physicians, nurses, rehabilitation teams, and families to create individualized care plans that support each patient's recovery journey. Key Responsibilities Guide patients and families through rehabilitation programs, insurance benefits, discharge planning, and patient rights. Coordinate education for patients, caregivers, and families to encourage active participation in treatment and recovery goals. Develop and implement individualized treatment and discharge plans. Complete psychosocial assessments and evaluate post-discharge needs. Collaborate with interdisciplinary teams, department leadership, and care providers to ensure seamless patient care. Advocate for patients while promoting HCA Healthcare's values of compassion, respect, and excellence. Qualifications Active Registered Nurse (RN) or Advanced Practice Registered Nurse (APRN) license. Associate Degree in Nursing or RN Diploma required. Minimum 2 years of recent Acute Care Case Management RN experience (within the last year). Benefits 401(k) with Company Match.
