We are currently seeking a Discharge Planner for Residential Services for Riveredge Hospital. Simply apply to the posting and discuss your interests with the hiring managers if you are selected to come in for an interview.
The Discharge Planneris responsible forthe coordination of the residents' case management and discharge needs. This includesassistingthe treatment team in the development ofappropriate dischargeplans,the facilitationof the attainment of resources to meet thisplanand the education of the residentsregardingthe discharge process. The discharge planner is a liaison between the treatment team andcommunity.
QualificationsExperience:
One (1) year of related experience, with knowledge of psychiatric patient care techniques with understanding of mental illness, preferred.
Bachelor Degree in psychology, social work, or related field preferred.
Additional Requirements:
Successful completion of CPR and CPI that is provided during orientation. May be required to work overtime and flexible hours.
