Home Health Start of Care Nurse

About the Role

We’re seeking a compassionate and detail‑oriented Start of Care Nurse to join our home health team. This role is critical in ensuring patients receive a smooth transition into care services, with thorough assessments and individualized care planning from day one.

Responsibilities

Conduct comprehensive patient assessments at the start of care
Develop and implement individualized care plans
Coordinate with physicians, caregivers, and interdisciplinary team members
Provide patient and family education on care procedures and health management
Ensure compliance with agency policies and regulatory standards
Document all assessments and interventions accurately and timely

Qualifications

Current RN license in
Minimum 1 year of nursing experience (home health preferred)
Strong clinical assessment and documentation skills
Excellent communication and interpersonal abilities
Ability to work independently and manage multiple priorities
Familiarity with EMR systems a plus
