What You'll Do As a CLASS Case Manager, you will provide comprehensive case management services to individuals enrolled in the Community Living Assistance Support Services (CLASS) Medicaid Waiver program, administered by the Texas Department of Aging and Disability Services (DADS). You will consistently meet all CLASS program standards as stated in the CLASS Provider Manual, Policy Revisions/Clarifications, and Information Letter Clarifications. Your Responsibilities Perform all required case management functions within specified timeframes. Maintain full, accurate and current documentation of evaluations, assessments, needs, progress, services, financial data, and all other required information in each individual’s record. Understand and subscribe to the philosophy and values of independent living, empowerment and community integration of all participants in the CLASS program and demonstrate same in all communications and actions. Empower CLASS clients to make informed decisions and support clients’ decisions regarding important aspects of life such as housing, employment, relationships, education, and other personal goals. Know each client in your caseload personally and be familiar with their goals, needs, opportunities and challenges. Meet regularly (no less than monthly) with clients and/or allies to discuss progress, problems, and plans and to monitor services provided according to the clients’ Individual Services Plan (ISP). Whenever possible, conduct such contacts where the participant lives, works, attends classes, or plays. Maintain cooperative working relationships with Direct Service Agencies (DSAs) and their representatives, including regular contact and consultation to enhance service delivery and assist the participant in achieving his/her individual goals. Respond to telephone calls in a timely fashion, always within 24 hours, and ensure clients have access to Case Management in emergency situations. Provide phone coverage when unavailable. Complete monthly program reports and program billing forms and submit them to the CLASS Program Director as required. Remain updated and knowledgeable about community resources and state and federal programs that relate to CLASS client needs. Qualifications A master’s degree in human services, drug and alcohol, education, counseling, psychology, or criminal justice, OR a Bachelor’s degree in a human behavioral science (with at least 30 semester hours or 45 quarter hours) in the relevant fields, with 2 years experience; or a high school diploma with 4 years of relevant experience. Minimum of two years of documentable experience providing services to children and/or adults with disabilities. Ability to learn to use Easter Seals’ Client Database expeditiously and independently. Spanish/English proficiency preferred. Travel throughout North Texas area to visit clients. Current driver’s license and proof of automobile insurance if travel is required. Compensation $16.12–18.00 per hour.
