Other duties as assigned. + Implements and monitors patient care plans to ensure effective treatment. Monitors, records, and communicates patient condition as appropriate. + Serves as the primary coordinator of all disciplines for well-coordinated patient care. + Notes and carries out physician and nursing orders accurately and promptly. Assesses and coordinates patient's discharge planning needs with members of the healthcare team. + Leads and mentors' staff to ensure optimal performance and competency in care delivery. + Conducts holistic, individualized assessments of each patient considering age, physiological, and psychological status. + Documents all phases of patient status while responsible for their care.
