Skilled Home Health Nurse

Schedule

This position operates on a Day shift, Monday to Friday, with varying hours between 8:00 AM to 6:00 PM. Alternating weekend shifts may also be required based on the needs of the HomeCare office.

Job Description

The Skilled Home Health Nurse will provide essential care to patients in their homes, focusing on cardiopulmonary assessments, wound care, and other medical needs. This position involves conducting skilled home visits, the typical daily workload consists of 6-8 visits, ensuring that patient care is delivered consistently and effectively.

Responsibilities

• Perform cardiopulmonary assessments and basic wound care.
• Change and care for foley catheters and perform staple/suture removal.
• Utilize glucometers and pro-time machines for patient monitoring.
• Administer IM and SQ injections, as well as lab draws via venipuncture.
• Provide IV skills, including PICC line or Midline care and dressing changes.
• Manage high-risk diagnoses, including heart failure, COPD, myocardial infarction, pneumonia, and stroke.
• Deliver post-operative care as needed.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home care experience is required.
• OASIS training and experience is necessary.
• Valid driver's license and the ability to use personal transportation for home visits.
• Must possess current BCLS certification.
• Must be COVID vaccinated or have an exemption on file, as well as a flu shot or declination with mandatory mask-wearing.
• Active Florida RN license or a compact license at the time of submission is required.
