Nurse Navigator (RN) - Medical Oncology

Role Overview: A Nurse Navigator is responsible for guiding oncology patients through diagnosis, treatment, and survivorship by coordinating care, providing education, and offering support. This role involves working closely with healthcare teams to identify high-risk patients, develop individualized care plans, and ensure seamless communication among patients, families, and providers. Key Responsibilities Proactively identify high-risk patients and conduct outreach via in-person and phone follow-ups. Coordinate appointments, diagnostic tests, and treatment plans. Serve as the primary communication link between patients, families, and care teams. Assist with medication refills, labs, imaging, and authorizations. Educate patients and families on disease management, navigating healthcare, and self-care strategies. Support clinical communication by triaging lab results, managing prescriptions, and addressing urgent issues. Participate in quality improvement initiatives and program development to enhance patient outcomes and comply with regulatory standards. Qualifications & Skills Bachelor of Science in Nursing (BSN) required. Current RN license and BLS certification; Oncology Certified Nurse (OCN) preferred. Strong knowledge of oncology care protocols, excellent communication, and interpersonal skills. Ability to manage multiple priorities, coordinate multidisciplinary care, and provide empathetic support.
