Part-Time Physical Therapist (K–8 School)

Christopher Columbus Charter School is seeking a part-time Physical Therapist to provide school-based physical therapy services to students in grades K–8. The Physical Therapist will evaluate students, develop and implement individualized therapy plans, participate in IEP meetings, and collaborate with teachers, families, and related service providers to support student success in the educational environment.

Qualifications:

• Current Pennsylvania Physical Therapist license.
• Pennsylvania Department of Education (PDE) Educational Specialist certification as a School Physical Therapist (or eligibility to obtain certification).
• Experience working with school-aged children, preferably in a K–8 setting.
• Strong communication, collaboration, and organizational skills.

2 - Copy of your DPW Child Abuse Clearance (Act 151)

3 - Copy of your FBI Clearance Fingerprinting with the UEID # (Act 114)

4 - Completion of a Sexual Misconduct/Abuse Disclosure Release (Act 168)

5 - Completion of PDE Form 6004: Arrest/Conviction Report and Certification (Act 24)

6 - Copy of high school and/or college transcripts

7 - Three letters of professional reference

8 - Completion of a PA School Health Record

9 - Completion of Mandated Reporter Abuse Training (Act 126)

10 - Completion of Professional Ethics Training
