Role Description

The Center Medical Director leads the clinical team at the Oak Street Health center, ensuring high‑quality patient care and a positive workplace culture. The role involves 80% clinical work in primary care and 20% administrative responsibilities, with mission‑critical leadership duties that set the standard for thoughtful primary care, mentor and coach providers, and participate in organization‑wide strategic initiatives.

Responsibilities

• Support organizational strategy by collaborating with the Practice Manager and Outreach Manager/Director to achieve objectives such as Best Care Anywhere, unmatched patient experience, exceptional growth and a great workplace. Drive implementation of OSH strategic initiatives and follow direction from regional leadership.
• Onboard new providers, model excellent medical care, and ensure care teams implement daily huddles, patient care planning, admission prevention, and documentation workflows. Teach and apply Oak Street's clinical guidelines and the value‑based full risk model. Assess and address root causes when center metric goals are not met.
• Create and maintain a culture of coaching and growth for providers and care teams. Listen to feedback, address concerns, conduct performance reviews, discuss provider goals, and develop plans to achieve those goals.
• Celebrate care team successes, communicate improvement opportunities, achieve triad best practices, and support a positive, joyful culture in practice.
• Other duties as assigned.

Qualifications

• M.D., D.O., A.P.N., or P.A. graduates with board certification in Internal Medicine or Family Medicine (required for M.D./D.O.) or in Family, Adult, or Gerontology (required for A.P.N.).
• 5+ years of outpatient practice experience, with a preference for fellowship training in Geriatrics.
• Proven clinical leadership experience, coaching providers, and a metrics‑driven approach to medical care and quality projects.
• Commitment to older adults and low‑income patients, and comfort with ambiguity and problem solving.
• Strong leadership skills, calm in high‑pressure situations, and a willingness to share impact with a fast‑growing team.
• US work authorization.

Salary & Benefits

Pay Range: $174,070.00 – $374,920.00 (annual, base only). The offer is based on experience, education, and geography.
