Experienced Caregiver Needed | Bellevue Nashville Area | Afternoon Shifts Delora Care, a Helping Hearts Company is seeking a dependable, experienced, and safety focused caregiver for a private home care assignment in the Bellevue Nashville, Tennessee area . We are looking for a caregiver who is mature, patient, observant, conscientious, dependable, and comfortable working independently in a client’s home. This is an excellent assignment for a caregiver who understands that quality caregiving is not only about completing tasks. It is about anticipating needs, maintaining a safe environment, protecting the client’s independence, and providing consistent, respectful support. Schedule This client is requesting care 7 days per week for approximately 3 hours per day during the afternoon . Available shift times may include 1:00 PM to 4:00 PM, 2:00 PM to 5:00 PM, or 3:00 PM to 6:00 PM . The schedule may include a combination of these times depending on caregiver availability and coverage needs. Applicants should clearly provide the days of the week and afternoon times they are available to work . About the Assignment The client is an adult recovering at home following a recent orthopedic procedure and currently uses a walker for mobility. The client is considered an EXTREME FALL RISK , so we are specifically looking for a caregiver who is highly attentive to fall prevention, safe transfers, proper ambulation assistance, and maintaining a clear and safe home environment. The caregiver must be comfortable providing hands on mobility and transfer assistance using a gait belt when required . The caregiver should understand proper body mechanics and be able to follow the client’s established mobility and safety plan. The client has a shower bench and requires standby assistance when getting into and out of the shower . The caregiver must remain attentive during shower entry and exit and understand that bathroom safety is especially important for this assignment. There are 9 exterior steps used to enter and exit the home , so the caregiver must be physically capable of safely navigating stairs and comfortable assisting a high fall risk client when mobility support is required. Responsibilities The caregiver will provide client focused, nonmedical personal support based on the client’s needs and preferences. Responsibilities may include assisting with safe ambulation and transfers, maintaining the walker within reach, providing standby shower assistance, preparing dinner, light housekeeping, laundry, changing bed linens, retrieving household items, grocery shopping, approved errands, watering plants, and assisting with basic pet care. The client already has a weekly professional housecleaning service. The caregiver is not being hired to perform deep cleaning . Caregivers are expected to keep the home tidy during the shift, clean up after activities and meals, and maintain clear walking areas to reduce fall hazards. The home has a cat. Applicants must be comfortable working around pets and understand that pet toys, bowls, supplies, and the animal itself can create additional trip hazards for a client with limited mobility. The ideal caregiver will notice reasonable tasks that need attention and complete them proactively rather than waiting for the client to provide constant direction. Transportation Requirements The client may require transportation for approved outings, appointments, grocery shopping, or errands. When transportation is provided, the client will travel in the caregiver’s vehicle, never the client’s vehicle . Any caregiver transporting the client must have a valid Tennessee driver’s license, current automobile insurance, reliable transportation, and a safe and properly maintained vehicle capable of transporting the client and her walker. Caregivers who do not currently meet the driving and automobile insurance requirements should not apply for this particular assignment if transportation will be required. Required Qualifications Applicants must be dependable, professional, safety conscious, and comfortable working with a client who is at very high risk for falls. Candidates should have caregiving experience and be comfortable with gait belt use, transfers, walker assistance, fall prevention, shower standby assistance, light housekeeping, meal preparation, errands, and supporting a client recovering at home. Applicants must also meet Helping Hearts employment and screening requirements, including successfully completing required background screening, Sex Offender Registry screening, and Abuse Registry screening . Caregivers assigned to this client must have reliable transportation and, when driving is required, maintain a valid Tennessee driver’s license and current automobile insurance throughout the assignment. Caregivers must be able to reliably report for scheduled shifts, communicate professionally with the office, follow the client’s written care plan, document services accurately, and promptly report any change in the client’s condition or safety. Documentation and Communication Caregivers are required to use WellSky Personal Care for required shift documentation and follow Helping Hearts procedures for clocking in, clocking out, and completing shift notes. Documentation must be completed in clear, complete sentences and accurately describe the care and services provided during the shift. Caregivers must communicate any fall, near fall, increased weakness, dizziness, significant mobility change, unsafe condition, or other client safety concern to the Helping Hearts office promptly. All scheduling and client related communication must go through the Helping Hearts office. Caregivers may not independently arrange schedules, additional hours, or service changes directly with the client or the client’s family. Caregivers may not independently contact the client’s family. Professional Expectations Helping Hearts caregivers are expected to arrive on time and ready to work for every scheduled shift. Caregivers must wear navy scrubs and closed toe shoes and maintain a clean, professional appearance. Caregivers must follow the client’s care plan, company policies, confidentiality requirements, and HIPAA requirements at all times. Client information, family information, photographs, medical information, addresses, schedules, and other private information may not be shared outside of authorized company communication. Caregivers should be calm, patient, respectful, and attentive while allowing the client to maintain as much independence as safely possible. What We Are Looking For We are especially interested in a caregiver who understands that this assignment requires strong fall prevention awareness and proactive safety support . The right caregiver will not rush the client, will keep walking paths clear, will remain attentive during transfers and ambulation, and will recognize when a task should be completed for the client to reduce unnecessary walking, bending, lifting, or prolonged standing. Consistency is very important. We are seeking caregivers who can commit to the days they accept and provide reliable ongoing coverage.
