Job Description Qualifications for Occupational Therapist - OT Home Health : Valid California Occupational Therapist (OT) license CPR Certification Reliable transportation and valid driver’s license with proof of insurance required. Responsibilities and Duties : Complete discharge planning by consulting with physicians, nurses, social workers, and other health care workers. Document patient care services via charting in a timely manner. Promote maximum independence by selecting and constructing therapies according to the individual's physical capacity and interest. Evaluate results of occupational therapy by observing, noting, and evaluating patient's progress; recommend and implement adjustments and modifications. Implement physician-ordered occupational therapies for self-care activities. Help the patient develop or regain physical or mental function, or adjust to their disability.
